What is your Immunization Policy?
Effective starting in the 2019-2020 school year, the Co-op no longer accepts personal, religious or philosophical exemptions. Only medical exemptions will be allowed as determined and authorized by the signature of a certified MD, ND, DO, ARNP or PA licensed under Title 18 RCW.
Parents will be required to provide up to date immunization records no later than Orientation Night (Sept). This will need to be an official Certificate of Immunization (CIS). You can request this document from your healthcare provider, or create an account on https://myirmobile.com/ to find and print your family’s immunization records yourself.
Handwritten immunization records will no longer be accepted. If you have additional questions regarding this, please contact our Registrar at registar@cwcoop.org
What immunizations are required?
Per Washington state law, children entering school, child care, or other early learning programs are required to have certain vaccinations before they can start. These requirements are put into place to protect children, families, and communities from vaccine preventable diseases. Immunization requirements apply to all enrolled children, including those learning remotely.
The parent or guardian of a child must provide documentation of vaccination or proof of immunity to specific diseases. This requirement is based on the national Advisory Committee on Immunization Practices (ACIP) Immunization Schedule for school or child care registration and attendance. The specific diseases are listed in the chart below:
